Negative Account Balances
Negative Account Balances The school district will make reasonable efforts to collect unpaid meal charges classified as delinquent debt. Families will be notified by an automated calling system and e-mail notification.
Negative balances of more than $50 not paid prior to the end of the month will be turned over to the Superintendent or Superintendent’s designee for collection. Negative balances of more than $15 not paid prior to the end of the school year will be turned over to the Superintendent or Superintendent’s designee for collection. Options for collection may include: collection agencies, small claims court, or any other legal method permitted by law.
Special Menu Requests
If your child has a medically-necessary dietary need, and your child will eat meals provided by the Nutrition Services Department, you will need to have your child’s doctor complete a Diet Modification Request Form. If you need a form, you can obtain one by printing off the forms below, calling the nurse or health associate of the school your child attends, or contacting the Waukee Nutrition Services Department at 515-987-2719. These forms will need to be signed by a physician licensed by the state (DO, MD, PA or ARNP only – Chiropractors are not accepted) and returned to the nurse where your student attends or the District Office Nutrition Department. New forms only need to be submitted if the accommodations your student receives need to change (example: better tolerance of dairy and student no longer needs to exclude cheese).

The Richard B. Russell National School Lunch Act requires the information on this application. You do not have to give the information, but if you do not, we cannot approve your child for free or reduced price meals. You must include the last four digits of the social security number of the adult household member who signs the application. The last four digits of the social security number is not required when you apply on behalf of a foster child or you list a Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF) Program or Food Distribution Program on Indian Reservations (FDPIR) case number or other FDPIR identifier for your child or when you indicate that the adult household member signing the application does not have a social security number. We will use your information to determine if your child is eligible for free or reduced price meals and for administration and enforcement of the lunch and breakfast programs. We MAY share your eligibility information with education, health, and nutrition programs to help them evaluate, fund, or determine benefits for their programs, auditors for program reviews, and law enforcement officials to help them look into violations of program rules.
