If your child has a medically necessary dietary need and will eat meals provided by the Nutrition Services Department, please have your child’s doctor complete a Diet Modification Request Form. You can print a form from the list below. If that’s not an option, please call your school’s nurse or contact the Waukee Nutrition Services Department at 515-987-2719.

These forms must be signed by an appropriately licensed provider (DO, MD, PA, ARNP, or Dentist. Chiropractors are not accepted.) and returned to the nurse or the District Office Nutrition Department. You only need to fill out a new form if something changes with your child’s accommodations. 

 

Special Notice Regarding Peanut & Tree Nut Allergens in School Meals

Nutrition Services does not use peanuts or tree nuts while producing food on-site. However, some purchased products are processed in a facility that also processes products with nuts. In this case, documentation is requested from the manufacturer regarding their procedures to prevent cross-contact. These products are listed below with a link to the manufacturer’s documentation.

If you want your child to eat an entire meal from a facility that is verified to be peanut/tree nut-free, have them select the SunButter sandwich, cheese stick, and crackers option, which is provided daily at our elementary schools.

Food items known to be processed in a facility with nuts are identified on the menu. Signs identifying these items are also posted in the service areas.
